Objectives:

At the end of presentation, the participant will be able to

1.  Define the term “population health”.

2. Describe drivers within the US health care delivery system toward value-based models of care.

3. Identify sources of data to define population health needs.

Continuing Education:

Nurses: The Georgia Board of Nursing deems Southwest Georgia Area Health Education Center (SOWEGA-AHEC) as an approved provider for nursing continuing education (CE). This activity is approved for 1.0 contact hour towards the continuing education competency requirement for Georgia nursing licensure renewal. No partial credit offered. Activity #2020-02a.

Submission of registration information, attendance and completed evaluation required for nursing continuing education certificates.
